If this will be your first appointment at my office (Davis
location), please arrive at least 5 minutes earlier than your
scheduled appointment to fill out your confidential client intake
form. If you would rather print this form out to fill out
before arriving at my office, you may do so by clicking the link
below. If you fill this form out before arriving, you do not
need to arrive early.
